Current and former employees may request a copy of their Form 1095-C.

Requests may be made by emailing sduff@furlowcharter.org, calling 229-931-8667, or requesting a copy in person/by mail from Stephanie Duff at 63 Valley Drive, Americus, GA 31709.

A copy of Form 1095-C will be provided within 30 days of the request.
